We have a 877 page software development book that we need converted to ePub format(s) suitable for posting to:

a) Amazon CreateSpace print on demand
b) iBooks Store
c) Kindle Store

The book is complete, created in InDesign, and we have a PDF version as well.  It contains the following formatting:
   
  * Pictures inline
  * Colored code blocks
  * Bolded text, and certain text formatted with a "code" font

We are looking for someone experienced with converting books made in InDesign to ePub good for CreateSpace, iBooks, and Kindle, with an eye to detail and professionalism.
